

LIFE CARE PLAN DEFINITION
The Life Care Plan is a tool constructed for the purpose of estimating
the medical and non-medical needs, with their associated costs, of a
person who has incurred a catastrophic injury. The Life Care Plan
addresses and projects the costs and frequency of needed goods
and services over an estimated life span, to include the necessary
equipment and assistive devices, replacement schedules and
maintenance and medical services that are anticipated to be required
by the injured individual.
The intention of the Life Care Plan is to address the client as a whole,
utilizing a team approach. The plan should serve as a guide, to follow
the client throughout their lifetime, to insure that funds will be
available to properly care for and provide for their needs. The Life
Care Plan is an estimate of needs, utilizing a team approach of
various health care providers offering their professional
insights/opinions based upon review of pertinent medical information
and in-person interviews with the injured individual and his family. The
nursing process, as defined by the American Nursing Association
guidelines and standards of practice, as well as the practice
guidelines and standards adopted by the American Association of
Nurse Life Care Planners and the Case Management Society of
America are employed in the preparation of the Plan.

Life Care Plan formats can be changed to meet your individual
needs. A complete Life Care Plan contains a narrative summary, an
assessment of needs, a life expectancy calculation based on the
CDC life tables and/or a physician/s assessment /evaluation and
tables projecting the cost of the needs over the estimated lifetime.
The format can be shortened or changed to meet your individual
needs and preferences.
